Im trying to find out what to do..i travel the highway to work and when driving over small bumps the rear of my car jerks. how can i fix this problem? i have a 2005 pontiac grand prix gt

I would recommend getting your shocks/struts and springs looked at.  You might have a strut that is goind bad.

May also be a rear toe problem. The Grand Prix (as well as it's GM stablemates) has issues with the rear control arms bending, especially if it's been lifted improperly. That will change the rear toe, if out enough, will cause the problem you describe.
